Jacques Memorial School Food Service Program
The Jacques Memorial School breakfast/lunch program is a prepaid system. Each student has a meal account in which parents make payments to cover their children’s purchases in advance: breakfast, lunch, or milk only. All accounts are OPEN unless we receive a letter from you requesting no purchases. From time to time, children do get adventurous and sometimes do make purchases that their parents are unaware of. Parents are responsible for these purchases.

Please note – there is a $3 charge per transaction for online meal payments. Payments to student accounts can also be made via cash or check (please make checks out to “Milford School District”).
SPECIAL DIETARY NEEDS INFORMATION/FORMS
Please refer to our menu linked above for selections. This will help you to determine the funds needed. Our menu is sent home monthly with your child and also available on the school website: jms.milfordk12.org.
Parents and Grandparents are welcome to join us. Please call 673-4434 ext. 5 by 9:00 am on the day you will be dining with us to be included in the lunch count. Adult meals are $4.00.

The breakfast and lunch program encourages healthy eating. We make every effort to provide healthy and satisfying foods for our students. Our menu selections include a variety of whole grains such as wheat, oats, bran, barley, and rice. We also use beef, pork, chicken, and turkey in our recipes. We offer fresh, frozen, or canned fruits and vegetables to encourage your children to try a wide variety of foods. A choice of whole milk, 1% low-fat milk or 1% chocolate milk comes with each meal or may be purchased individually.

We ask parents to contact us to discuss your child’s needs. Please contact the school nurse 673-4434 extension 2 or the Kitchen at 673-4434, extension 5, so we can help you with your food options.
How to make a payment
All checks or cash should be in an envelope with the student’s full name and teacher’s name to ensure proper credit. Payment is due every FRIDAY for the upcoming week. Some parents prefer to mail their checks to us directly so payments are not lost. Money stays in your child’s account until it is used, so absences and field trips need not be a worry. Parents are asked to keep sufficient money in their child’s account. Please call us to check your balance. Low balance notices will be sent home with the children so please check their backpack daily; if you receive a notice, payment should be sent in the next day. How do I apply for Free or Reduced assistance?
Parents need to fill out assistance forms each year. All information is kept strictly confidential. Applications are sent home with every child the first day of school and are processed as quickly as possible. Letters to notify parents of acceptance are mailed home. Forms are always available anytime during the school year, and can be found by clicking the link at the top of this page. Parents who need help with free or reduced applications or have any questions or concerns are asked to contact Cathy Casali, Bookkeeper, at 673-4201 ext 3308.
